Nov 23, Author: A prolonged aPTT result may indicate the following [ 12 ]:. A shortened aPTT result may indicate the following [ 12 ]:. Many drugs can change the results of the activated partial thromboplastin time aPTTincluding nonprescription drugs.

No specific preparation is required. However, since lipemia may interfere with photoelectric measurements of clot formation, specimens should not be obtained after a meal. If the patient is receiving heparin by intermittent injection, Thrombophlebitis Geschichte, the sample should be drawn minutes before the next dose, while, if the patient is receiving a continuous heparin infusion, the sample can be drawn at any time.

Factors that interfere with the aPTT test are include the following [ 12 ]:. Usually, 5 mL of citrated, platelet-poor plasma is prepared from 4, Thrombophlebitis Geschichte.

After collection, the whole blood sample is centrifuged, Thrombophlebitis Geschichte, and the plasma is removed and placed in another blue-top tube, Thrombophlebitis Geschichte. Related tests include the following [ 15 ]:. Partial thromboplastin time PTT and activated partial thromboplastin time aPTT are used to test for the same functions; however, in aPTT, an activator is added that speeds up the clotting time and results in a narrower reference range.

The aPTT test is used to measure and evaluate all the clotting factors of the intrinsic and common pathways of the clotting cascade by measuring the time in seconds it takes a clot to form after adding calcium and phospholipid emulsion to a plasma sample.

The result is always compared to a Thrombophlebitis Geschichte sample of normal blood, Thrombophlebitis Geschichte. When the aPTT test is performed in conjunction with prothrombin time PT test, which is used to evaluate the extrinsic and common pathways of the coagulation cascade, a further clarification of coagulation defects is possible.

Normal hemostasis is Thrombophlebitis Geschichte when there is a balance between factors that encourages clotting and factors that encourages clot dissolution.

Following Thrombophlebitis Geschichte to a blood vessel, the first reaction of the body is vascular constriction to reduce blood loss. In small-vessel injury, Thrombophlebitis Geschichte, this may be enough to stop bleeding. However, for large blood vessels, hemostasis Varizen behandelt mit Jod Thrombophlebitis Geschichte. Primary hemostasis occurs within seconds and results in platelet plug formation at sites of injury.

Next, secondary hemostasis occurs, which consists of the reactions of the plasma coagulation system that result in fibrin formation. It requires several minutes for completion.

The fibrin strands that are produced strengthen the primary hemostatic plug. In the first phase of reactions, called the intrinsic system, 3 plasma proteins, Thrombophlebitis Geschichte, Hageman factor factor XIIhigh-molecular-weight kininogen, and prekallikrein, form a complex on vascular subendothelial collagen, and, through a series of reactions, activated factor XI XIa is formed and activates factor IX IXa.

At the same time, the extrinsic system is activated and provides a second pathway to initiate coagulation by activating factor VII VIIa. Alternatively, both factors IX and X can be activated more directly by factor VIIa, generated via the extrinsic pathway. Activation of factors IX and X provides a link between the intrinsic and extrinsic coagulation pathways.

The final step, the common pathway, converts prothrombin II to thrombin IIa in the presence of activated V Vaactivated X Xacalcium, Thrombophlebitis Geschichte phospholipid. The main purpose of thrombin IIa is the conversion of fibrinogen to fibrin, Thrombophlebitis Geschichte, which is then polymerized into an insoluble gel.

The fibrin polymer is then stabilized by the cross-linking of fibrin polymers by factor XIII. Clot lysis and vessel repair begin immediately after the formation of the definitive hemostatic plug. Three potential activators of the fibrinolytic system, Hageman factor fragments, urinary plasminogen activator, and tissue plasminogen activator, diffuse from endothelial cells and convert plasminogen, which had previously been adsorbed to the fibrin clot, into plasmin.

Plasmin then degrades fibrin polymer into small fragments, Thrombophlebitis Geschichte, Thrombophlebitis Geschichte are cleared by the macrophages. Because factors II, IX, Thrombophlebitis Geschichte, and X are vitamin K—dependent, biliary obstruction, which precludes gastrointestinal absorption of fat and fat-soluble vitamins including vitamin Kcan reduce their concentrations and thus prolong the aPTT.

Thrombophlebitis Geschichte aPTT is usually followed by mixing studies when the cause is not obvious, eg, due to heparin contamination or to other preanalytical problems such as an insufficient or clotted blood sample to evaluate for deficient coagulation Thrombophlebitis Geschichte s or a coagulation inhibitor s.

If mixing the two plasma samples corrects the aPTT Thrombophlebitis Geschichte, there is clotting factor deficiency, Thrombophlebitis Geschichte, and specific coagulation factor testing is performed to determine which factor s is deficient.

If the Thrombophlebitis Geschichte fails to correct the aPTT results within seconds, it strongly suggests 1 a coagulation factor inhibitor eg, an acquired factor VIII antibody or 2 an antiphospholipid antibody or lupus anticoagulant a nonspecific inhibitor.

In this case, the aPTT result Thrombophlebitis Geschichte not correct with normal plasma mixing but it will usually correct if an excess of phospholipid is added to the sample. If lupus anticoagulant is suspected, a more sensitive test, lupus anticoagulant—sensitive aPTT or Dilute Russell Viper Venom Test, Thrombophlebitis Geschichte, should be performed, Thrombophlebitis Geschichte.

Reference Range Partial thromboplastin time PTT and activated partial thromboplastin time aPTT are used to test for the same functions; however, Thrombophlebitis Geschichte, in aPTT, an activator is added that speeds up the clotting time and results in a narrower reference range.

The reference range of the aPTT is seconds, Thrombophlebitis Geschichte. More Thrombophlebitis Geschichte 70 seconds signifies spontaneous bleeding. More than seconds signifies spontaneous bleeding [ 1Thrombophlebitis Geschichte, 2 ]. Interpretation A prolonged aPTT result may indicate the following [ 12 ]: Congenital deficiency of Fitzgerald factor prekallikrein.

Von Willebrand diseasewhich is the most common inherited bleeding disorder, affecting platelet function owing to decreased von Willebrand factor activity. Liver cirrhosis the liver makes most of the clotting factors, including those that are vitamin K-dependent ones ; diseases of the liver may result Thrombophlebitis Geschichte an inadequate quantity of clotting factors, prolonging the aPTT.

The synthesis of some clotting factors requires vitamin K, so vitamin K deficiency results in an inadequate Thrombophlebitis Geschichte of intrinsic system and common pathways clotting factors, as a result the aPTT is prolonged. Disseminated intravascular coagulation DIC: The clotting factors involved in the intrinsic pathway are consumed, prolonging the aPTT.

Heparin therapy, which inhibits the intrinsic pathway at several points eg, prothrombin IIprolonging the aPTT [ 3 ]. Nonspecific inhibitors, such as lupus anticoagulant and anticardiolipin antibodies, which bind to phospholipids on the surface of platelets. Specific circulating anticoagulants, inhibitor antibodies that specifically target certain coagulation factor, such as in individuals with hemophilia after many plasma transfusions, systemic lupus erythematosusrheumatoid arthritistuberculosisand chronic glomerulonephritis.

Early stages of DIC: Extensive cancer eg, ovarian cancerpancreatic cancercolon cancer. An acute-phase response leading to high factor VIII levels. Collection and Panels Many drugs can change the results of the activated partial thromboplastin time aPTTincluding nonprescription drugs. Drugs that may prolong the test values, including antihistamines, ascorbic acid, chlorpromazine, heparin, Thrombophlebitis Geschichte, and salicylates.

Hematocrit that is highly increased or decreased. Blood samples drawn from heparin lock or a heparinized catheter. Background Description Partial thromboplastin time PTT and activated partial thromboplastin time aPTT are used to test for the same functions; however, in aPTT, an activator is added that speeds up the clotting time and results in a narrower reference range.

Thrombotic episode or recurrent miscarriages. To evaluate the effectiveness of drug therapy eg, unfractionated heparin therapy. To assess the integrity of the intrinsic and final common pathways.

Previous article in issue: Next article in issue: Diagnostic management of acute deep vein thrombosis and pulmonary embolism. Deep vein thrombosis DVT is a common disease. However, unlike that of varicose veins, which have been depicted since antiquity in art and literature, its description was more recent in the history of medicine.

The number of reported Thrombophlebitis Geschichte cases steadily increased thereafter, particularly in pregnant and postpartum women, Thrombophlebitis Geschichte. During the first half of the 20th century, Thrombophlebitis Geschichte, well before the discovery of anticoagulants, many therapeutic approaches were used, and arose from the pathologic hypotheses that prevailed at their time.

Most of these treatments have now been abandoned, or even forgotten. We also bring some perspective to historical aspects of this disease and its treatment, notably regarding elastic compression and early mobilization, but also abandoned and complementary treatments.

In these times of change regarding DVT treatment, mainly marked by the arrival of new oral anticoagulants, efforts of physicians through the ages to treat this common disease provide a beautiful example of the history of knowledge, Thrombophlebitis Geschichte.

This development opens a new era in the management of venous thromboembolic disease [ 2 ]. The developments in DVT treatment and the efforts of physicians through history to treat this common disease provide an illuminating example of the progress Thrombophlebitis Geschichte knowledge, Thrombophlebitis Geschichte. It is of interest to look back at the history of DVT treatment.

Unlike previous reviews, this historical analysis will not focus on the gradual improvement in the anticoagulant drugs available. We will not address the treatment of PE, which has its own history and has been reviewed separately, or the history of new oral anticoagulant agents, Thrombophlebitis Geschichte, which is just beginning [ 7 ].

Unlike the situation for varicose veins, leg ulcers, or lower limb edema, to which there are extensive references in antique art and literature, the first description of a case truly compatible with a DVT first appears during the Middle Ages [ 8, 9 ].

Raoul's surgeon, Henri de Perche, advised him to wait and see. Unfortunately, the patient's symptoms worsened, and he developed a leg ulcer, Thrombophlebitis Geschichte. He visited St Sauber Beine Creme Krampf Preis und Bewertungen shrine, without any improvement. After additional unsuccessful unspecified treatment attempts, Raoul was advised to visit the tomb of King Saint Louis.

He spent several days praying to the saint, and then he decided to collect the dust that he found below the stone that covered the tomb, and he applied it directly to the ulcer. Thus, this first reported case of effective treatment of DVT might not be the most reproducible.

In antiquity, neither famous Greek physicians such as Hippocrates and Oribasius, nor Roman physicians such as Galen or Caelius Aurelianus, reported a case suggesting a diagnosis of DVT; this is also true of the art of ancient Egypt, Persia, and South America, Thrombophlebitis Geschichte. Nevertheless, it is likely that this disease is as old as humankind, and it may have affected famous historical figures: Thus, although venous thrombosis is a frequent disease, it appears that no cases clearly compatible with the diagnosis of DVT were reported before the description of the case of Raoul.

Raoul, the first case of deep vein thrombosis [ 9 ]. After this first unquestionable description by Guillaume de Saint Pathus, the number of reported cases of Thrombophlebitis Geschichte increased rapidly, Thrombophlebitis Geschichte, and the first pathologic hypotheses arose, leading to the first treatment attempts.

Thus, in the late s, breast-feeding was encouraged to prevent DVT [ ], Thrombophlebitis Geschichte. This technique was used to treat DVT and many other diseases until the end of the 19th century [ 20, 21 ].

From the 17th century, Thrombophlebitis Geschichte, the humoral theory, which was Thrombophlebitis Geschichte used to explain DVT pathophysiology, was gradually abandoned, Thrombophlebitis Geschichte. InWiseman suggested that DVT was the consequence of an alteration of blood, Thrombophlebitis Geschichte, and then, inHunter hypothesized that it was an occlusion of the vein by blood clots [ 15 ].

Inwell before Virchow demonstrated the relationship between DVT and fatal PEThrombophlebitis Geschichte, Hunter had performed venous ligations above thromboses, to prevent extension of clots [ 22, 23 ]. In the absence of any other truly effective treatment for preventing fatal PE, this technique became more widely used at the end of the 19th century [ 24 ].

The ligation could be performed at the femoral, common femoral, iliac and inferior vena cava IVC levels in cases of proximal thromboses, and more rarely in cases of distal thromboses, although this latter therapeutic intervention remained controversial [ 25, 26 ]. This surgical treatment was still widely used until the midth century in association with, or instead of, anticoagulants [ 7, 27 ].

For fear of thrombus migration, strict bed rest was prescribed, and constituted, at least from the end Krampfadern der Hoden von Männern the 19th century, the cornerstone of DVT treatment.

Thus, in cases of DVT, the patient's lower limbs were set in iron splints to prevent any movement Fig. Special, reclining, orthopedic beds were also used to favor venous return Fig. Devices used to favor strict immobilization for cases of deep vein thrombosis France, early 20th century [ 28 ].

B Reclining iron bed. This was consistent with the observation that DVT is, in many cases, associated with fever, and Thrombophlebitis Geschichte occurred Thrombophlebitis Geschichte, after — septic — surgical procedures, or during bed rest for an infectious disease.

Consequently, the treatments prescribed involved anti-inflammatory medication and the prevention and treatment of infection [ 20, 29 ], Thrombophlebitis Geschichte. Bloodletting, sometimes via application of leeches, which was popular in these times, was thought to be one of the most effective methods to relieve lower limb congestion and to treat inflammation [ 20, 32 ].

Apart from bloodletting, Thrombophlebitis Geschichte, other therapeutic options consisted of: These therapeutic attitudes, based on the theory of inflammation and infection, started to become Fliegenpilz gegen die Creme gegen Krampfadern in the early s.

Overall, prior to the s, before the introduction of Thrombophlebitis Geschichte, the most common treatment for DVT mainly relied on: Because major risk factors for DVT had already been identified, most von dem, was früher Krampfadern used thromboprophylactic measures were already known and applied in hospitalized cases. For example, Wright describes prophylactic measures such as early ambulation, elastic compression, and avoidance of dehydration and of tightly applied adhesive strapping [ 22 ], Thrombophlebitis Geschichte.

By the middle of the 19th century, the major pathologic mechanisms of venous thrombosis had been discovered. They were first summarized in the famous Virchow's triadtheorized by Andral in and which Virchow probably never described [ 36 ]. However, it was only towards the s that a consensus appeared regarding the three factors contributing to thrombosis: During this period, a number of therapeutic breakthroughs, Thrombophlebitis Geschichte, most of Thrombophlebitis Geschichte discovered by accident, Thrombophlebitis Geschichte DVT treatment.

The first isolated anticoagulant was hirudin, obtained in by Haycraft [ 32 ]. It was extracted from the saliva of leeches. However, it could not be used as a potent anticoagulant agent until its production by genetic engineering in Thus, the first anticoagulant that could be effectively used for the treatment of DVT was heparin.

It was discovered in by McLean, at that time a medical student, while he was studying the procoagulant properties of crude ether and alcohol extracts of the brain, liver and Thrombophlebitis Geschichte under the supervision of Howell [ 37 ]. McLean noticed that these extracts, and more particularly heparphosphatide, Thrombophlebitis Geschichte, became anticoagulant agents after long-term exposure to air [ 37 ].

Four years later, he discovered true heparin, a water-soluble mucopolysaccharide from dog liver, Thrombophlebitis Geschichte. A dispute over the paternity of the discovery started between the two scientists. However, this dispute might well have been vain, as it emerged later that the anticoagulant isolated years earlier in by Doyon after peptone shock was, in fact, heparin [ 3 ]. InCharles and Scott succeeded in producing pure crystalline heparin, allowing its use in humans, which began in [ 38 ].

The severity of DVT and the absence of convincing effective treatment were probably the main reasons for the rapid spread of this innovative drug. As was the case for today's modern anticoagulants, Thrombophlebitis Geschichte, the first indication for heparin was thromboprophylaxis in surgical patients: Despite the absence of randomized placebo-controlled trials, the effectiveness of heparin was almost immediately considered to be unquestionable: Therefore, in the early s, heparin was already widely used, when available.

A mysterious hemorrhagic disease decimated cattle in the area, ruining numerous farmers [ 4, 43 ]. InSchoefield, a Canadian veterinary pathologist, showed that the disease was caused by spoiled sweet clover [ 3 ]. It could be prevented by withdrawal of the Thrombophlebitis Geschichte clover from food, and Thrombophlebitis Geschichte be treated by blood transfusion.

Two years later, dicoumarol was used for the first time to treat DVTs [ 44 ]. The proliferation of rats was a public health concern; rodent behavior, and particularly their habit of eating small pieces of foodstuff and the presence of tasters among them, made eradication by fast-acting poisons poorly effective.

Link decided to test the anticoagulant power of all coumarins that had been synthesized in Thrombophlebitis Geschichte laboratory between and to develop the optimal rodenticide, Thrombophlebitis Geschichte. Here started the story of warfarin, initially launched in as the ideal rat poison and considered to be too toxic for human use Fig.

This opened the way to its commercialization as a therapeutic agent in [ Thrombophlebitis Geschichte ]. Physicians Thrombophlebitis Geschichte realized that the two drugs were complementary: Warfarin, its discoverer, and its original indication warfarin, discovered by Link, was initially launched in as the ideal rodenticide, and was considered to be too toxic for human use [ 43 ]. The most important advances in the field of DVT therapeutic management occurred during the first half of the 20th century.

However, numerous other therapeutic options, sometimes surprising, were tried during this period and later abandoned because of insufficient efficacy: This last technique, Thrombophlebitis Geschichte, developed by Debakey inwas used during the s, Thrombophlebitis Geschichte. The underlying pathophysiologic rationale for the use of lumbar sympathic block was based on images of venographic series, which suggested that DVT was accompanied by severe vasospasm [ 22, 25 ].

Another debated issue, which arose around the s, Thrombophlebitis Geschichte, concerned whether or not patients should be mobilized during the acute phase of DVT [ 22, 34 ]. This question was resolved only subsequently, half a century later, thanks to the development of low molecular weight heparin LMWH. This has dramatically modified DVT presentation and management. Indeed, thanks to venography, which, although developed by Berberich and Hirsch inhas been widely used only since the s after the standardization of the procedure, physicians no longer treat clinically suspected DVT but objectively confirmed DVT [ 5, 45 ].

DVT is also treated earlier, and is even diagnosed and treated while clinically asymptomatic. From a therapeutic point of view, Thrombophlebitis Geschichte, this period has seen the simplification of anticoagulant treatment and the end Thrombophlebitis Geschichte the bed-rest dogma, which allowed the emergence of home treatment and the development of complementary treatments to decrease both mortality in high-risk patients and the burden of long-term Thrombophlebitis Geschichte. This attitude was based on pathophysiologic data.

The duration of bed rest was also based Thrombophlebitis Geschichte more practical Thrombophlebitis Geschichte However, prolonged immobilization was often associated with serious adverse consequences, including ankle or knee ankylosis, and amyotrophia [ 29, 30, 46 ]. In the s, a growing number of authors started to raise serious doubts about this management, and pleaded in favor of early ambulation and the use of compression stockings to fix the thrombus in place [ 22, 34 ].

However, in the absence of clear evidence of its harmlessness, most physicians remained reluctant to recommend immediate mobilization, for fear of thrombus migration. In addition, Thrombophlebitis Geschichte, the need to administer continuous intravenous infusions of heparin was a major impediment to early ambulation. The most significant step in the simplification of anticoagulant treatment was the development of LMWH, which, in most cases, does not require Thrombophlebitis Geschichte. InLevine demonstrated that LMWH given at home was as safe and effective as Thrombophlebitis Geschichte heparin administered in the hospital to treat proximal DVT [ 49 ].

In the same year, Partsch's small randomized trial provided evidence that, relative to bed rest, Thrombophlebitis Geschichte, early ambulation with compression stockings improved pain and counteracted swelling without increasing the risk of PE [ 50 ], Thrombophlebitis Geschichte. These studies Thrombophlebitis Geschichte rapidly followed by widespread implementation Thrombophlebitis Geschichte early ambulation with compression stockings in outpatient Thrombophlebitis Geschichte, which has now become the standard and, indeed, Thrombophlebitis Geschichte, recommended management [ 1, Thrombophlebitis Geschichte, 55 ].

Thus, DVT became an ambulatory disease. The era of novel anticoagulants could start.
